0.2% of patients on neuroleptic drugs, particularly potent dopaminergic antagonists such as heloperidol.
Symptoms appear a few days to a few weeks after initiation of therapy and consist of:
*hyperthermia
*muscle rigidity
*autonomic instability (tachycardia, labile BP, pallor)
*fluctuating level of consciousness.
Investigations show:*increased creatinine phosphokinase
*increase WCC
*abnormal liver biochemistry.
Treatment can be:(a) Specificbromocriptin--to enhance dopaminergic accuracy
dantrolene--to reduce muscle tone
(b) Nonspecificgeneral management including temperature reduction.
