כאב הוא תחושת סבל ואי-נוחות, המחווה על פגיעה ברקמות הגוף וממוקמת באזור מסוים בו. לפי האגודה הבינלאומית למחקר כאב, כאב מוגדר כ"תחושה וחוויה נפשית לא נעימה הקשורה בנזק ממשי או אפשרי לרקמה, או המתוארת במונחים של נזק מסוג זה". בשל ההבנה חוצת התרבויות כי כאב נקשר בפגיעה גופנית, ומאחר שהוא עלול לפגוע במידה ניכרת באיכות החיים של הפרט, זהו התסמין הנפוץ ביותר המביא לפנייה לרופא.

עם זאת, יתרונו האבולוציוני של הכאב הוא בכך שהוא מתריע בפני האדם או בעל החיים על סכנה מידית לפגיעה גופנית, או על נזקים ארוכי טווח צפויים, ועל כן הוא דוחק בפרט להימנע מן הסכנה ולקרוא לעזרה. נוסף על כך, הוא מסייע בהחלמה מפציעה בכך שהוא מאלץ מנוחה לאיבר הפגוע, ומלבד תרומתו לפרט הוא גם מועיל לקולקטיב, בכך שתשדורת המצוקה מצד הפרט מעודדת את הקולקטיב לנקוט אמצעי זהירות כהגנה מפני הסכנה.

תחושת הכאב הגופני טומנת בחובה גם היבט של סבל רגשי, ולא זו בלבד, אלא שככל שגוברת עצמת הכאב, כך הכאב מלווה בחרדה ודחף להפסיק את תחושת הכאב. דבר זה מגדיר את הכאב הן כתחושה והן כרגש. כאב, במיוחד כאב חריף, הוא גורם דחק המוביל לעוררות ולתגובה גופנית אופיינית המתבטאת, למשל, בעליית לחץ הדם והדופק.
